In this vital role, you will oversee the comprehensive coordination of personalized patient care, working closely with multidisciplinary teams to ensure that patient care plans are executed seamlessly across various settings. The ideal candidate will be a compassionate healthcare professional who combines strong clinical understanding with outstanding communication and organizational skills to improve patient outcomes and enhance the efficiency of healthcare delivery. Assess patient health needs and develop comprehensive care plans in collaboration with physicians and other healthcare providers. Monitor and ensure the implementation of these plans, making adjustments as patient needs evolve. Advocate for patients and their families, providing guidance and resources to ensure quality care. Manage a caseload of patients, maintaining up-to-date records and communicating key information to relevant team members. Coordinate with insurance providers to secure necessary coverage and confirm that treatments meet policy standards. Strong clinical knowledge and skill in a healthcare setting Excellent organizational and communication skills Ability to work effectively with a multidisciplinary team Strong problem-solving skills and the ability to make sound decisions Proficiency with medical software and documentation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license Certification in Case Management (CCM) preferred but not required
