Job Description:

Sanford Health is seeking a dedicated RN Case Manager to join our vast healthcare network and contribute to our mission of delivering exceptional medical care. In this role, you will oversee a continuum of care for patients from admission to discharge, ensuring optimal outcomes through efficient, cost-effective care coordination. With a focus on patient-centered plans, you will collaborate closely with medical teams across various departments to tailor treatment plans to the individual needs of patients. Your contributions will be crucial in enhancing patient satisfaction, reducing hospital readmission rates, and supporting the holistic well-being of those under our care. Assess new patients, identifying their needs and expected care outcomes. Develop, implement, and evaluate individualized patient care plans. Facilitate multi-disciplinary team meetings to review treatment plans and patient progress. Act as a liaison between patients, families, and healthcare professionals to ensure clear communication and effective treatment. Monitor patient progress and adjust care plans as necessary. Educate patients and families on treatment expectations, disease management, and prevention strategies. Ensure compliance with healthcare regulations and standards. Strong interpersonal and communication skills. Proficient in clinical assessment techniques and care planning. Ability to manage multiple cases effectively with attention to detail. Experience in utilization review, quality assurance, and discharge planning. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license in the state of practice. Certification in Case Management (e.g., Commission for Case Manager Certification) preferred.
