Sanford Health, a leading health system known for comprehensive care and advanced medical practices, is seeking a skilled RN Case Manager to join our dedicated team. In this role, the RN Case Manager will be pivotal in collaborating across multiple healthcare settings to ensure continuity of care, optimize patient outcomes, and enhance the effectiveness of healthcare services. Based at our modern facilities, this position involves direct patient interaction, multidisciplinary team coordination, and case management, including assessment, planning, implementation, and evaluation of patient care. Conduct comprehensive patient assessments to identify health needs and barriers. Develop, implement, and evaluate individualized care plans in collaboration with a multidisciplinary healthcare team. Facilitate communication between patients, family members, and healthcare professionals to ensure clarity and continuity of care. Monitor patient progress and outcomes, adjusting care plans as necessary. Provide education and resources to patients and families to support health management and decision making. Ensure compliance with healthcare regulations and standards. Strong clinical judgment Proficient in case management software and medical record systems Excellent communication and interpersonal skills Ability to manage multiple cases simultaneously with high attention to detail Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Registered Nurse (RN) license Certification in Case Management (CCM) preferred
