Job Description:

Sanford Health is seeking a dedicated and compassionate RN Case Manager to join our healthcare team, spread across multiple facilities in the United States. This position will play a critical role in coordinating comprehensive care for patients, focusing on their overall health needs while seamlessly integrating services across our diverse medical environments. As an RN Case Manager at Sanford Health, you will ensure the delivery of quality care by assessing patient needs, developing effective care plans, collaborating with healthcare professionals, and advocating for patient welfare. Assess patient needs and develop individualized care plans. Coordinate with healthcare providers to streamline various services for patients. Monitor and evaluate the effectiveness of the care plan and make adjustments as necessary. Educate patients and their families about their conditions and the care needed. Advocate for patient needs and act as a liaison between patients and physicians. Ensure compliance with healthcare laws, policies, and standards. Strong understanding of clinical practices and principles. Excellent communication and interpersonal skills. Ability to manage multiple cases effectively. Strong organizational and problem-solving skills. Proficiency in electronic health record (EHR) software. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Registered Nurse (RN) license. Certification in Case Management (CCM) preferred.
