Job Description:

Good Samaritan Health Center seeks a dedicated Physical Therapist (PT) to join our team and contribute to our mission of delivering holistic health services to underserved communities. Our center emphasizes a comprehensive approach to physical therapy, integrating it within broader wellness goals tailored to our patient base. The successful candidate will be deeply involved in developing individualized patient care plans that foster recovery, improve mobility, and enhance overall quality of life, all while addressing the unique needs of our diverse community members. Evaluate patient conditions based on their health needs and medical histories. Create and execute patient-centered, individualized treatment plans based on physical assessment and diagnosis. Engage and educate patients and their families on continuing with effective home exercises. Maintain patient records and document patient care services. Collaborate with healthcare professionals to maximize patient recovery and health outcomes. Stay updated with advancements in physical therapy and integrate them into patient care. Strong interpersonal and communication skills Ability to formulate detailed, efficient, and adaptive patient care plans Proficiency in using physical therapy software and equipment Strong observation skills to diagnose patient needs and assess treatments effectiveness Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed to practice as a Physical Therapist in the state CPR certified Membership in the American Physical Therapy Association (APTA) preferred
