Job Description:
Job Description Job Description We are looking for a committed
individual to join our family. This full-time position is 40 hours
per week with full benefits to include Health, Dental, Vision,

available. As MDS Coordinator your schedule would consist of
Monday-Friday with clinical on-call every three weeks and a weekend
rotation approximately every 12 weeks. DIRECT HIRE ONLY Shift
Information: 8a-430p M-F Call every 3rd week FTE: 40 Hour Per week
Patient Population: Skilled Nursing Required Education: Registered
Nurse Desired Skills: RN with a minimum of 1 year of experience
working as an RN in Long Term care setting with knowledge of MDS,
Medicare and care planning processes Base pay begins at
$34.50/hour, with maximum wage determined by experience. Sign On
Bonus; $15,000 Responsibilities: Administer nursing care to ill,
injured, or disabled patients Diagnose and establish patient
treatment plans Monitor and report changes in patient symptoms or
behavior Communicate with collaborating physicians or specialists
regarding patient care Educate patients about health maintenance
and disease prevention Facilitate referrals to other healthcare
professionals and medical facilities Maintain accurate patient
medical records Provide advice and emotional support to patients
and their family members Qualifications: RN license for state of
practice. RN with a minimum of 1 year experience working as RN in
LTC setting Familiarity with medical software and equipment Ability
to build rapport with patients Strong problem solving and critical
thinking skills Ability to thrive in a fast-paced environment
