- Wellness Program Responsibilities CHI St. Alexius Palliative
Care is looking for a Full Time RN Health Coach to join the team.--
The RN Health Coach Palliative Care works with patients and
families, using education, goal setting, action planning, problem
solving and motivational interviewing to support the patient and
family during the course of serious medical illness. What You'll
Do:
- Effectively utilizes the nursing process to manage patients
with chronic disease within the Palliative Care Clinic--
- Assumes responsibility and accountability for care plan
effectiveness and client outcome.--
- Assesses medication adherence when patient specific goals are
not met.--
- Based on collaboration with patient and assessment, defines
nursing diagnosis and plan of care or communicates with provider
for medical advice and plan of care.--
- Implements care plan.--
- Work with patients and families on self-management support
including short and long term goal setting for self-management of
chronic disease and health behavior change and coordinates their
care across the continuum.
- Screens potential patients for appropriate entry into the
palliative care program.--
- Initiates and follows through with interdisciplinary care
team.--
- Collaborates with the primary care provider, the palliative
care team, and other members of the healthcare team to engage the
clients in health promotion and health care maintenance needs (i.e.
smoking cessation, immunization needs).------
- Continuously evaluates patient progress, readjusts
communication method with patient, pursues goal setting, evaluates
and adjusts plan of care accordingly.--
- Partners with the clinic and/or family to determine their
perceptions of priority health care progress toward established
goals utilizing structured care methodologies.--
- Assesses the client's emotional status, cognitive status, and
learning readiness within the client's environment to augment the
holistic plan of care.--
- Facilitates mutual goal setting and goal-directed interactions
with the client/family.
- Collaborates with community resource persons to deliver
services that promote quality outcomes and continuity of
care.--
- Assumes the patient advocacy role to identify and resolve
issues within the healthcare delivery system. Collaborates
routinely with primary provider (MD's) and the interdisciplinary
team regarding the client's progress and changing needs/
desires.----
- Provides for safe patient transitions and hand offs through
effective communication with patient, family and RN/or other care
provider - including the patient and family in communications
related to their cares.---- Qualifications Education: Bachelor's
Degree in nursing preferred.-- Licensure: Currently licensed with
the applicable state nursing board and/or possess--multistate
licensure privileges. Functions within the legal scope of practice.
Must hold a valid-- drivers license. Experience:--Minimum of three
years of professional nursing experience in a clinical care setting
preferred.-- The RN Health Coach Palliative Care must attend
Palliative Care training and obtain certificate of competency or
attain palliative care certification at the earliest opportunity,
not to exceed 2 years. Pay Range $24.89 - $35.15 /hour
