Job Description:
DescriptionThe Community Program Manager will implement the
Alzheimer's Association's best practices to achieve growth goals
and benchmarks by increasing awareness of Alzheimer's and other
forms of dementia, delivering care and support program activities,
growing partnerships with organizations, and recruiting and
mentoring care and support volunteers. This position focuses
strongly on reaching underrepresented populations, including rural
and Tribal communities and will partner with all other mission
pillars of the Alzheimer's Association (Diversity, Equity, and
Inclusion, Public Policy, Development, Concern and Awareness,
Health Systems and Operations) to assist in reaching full mission
strategic goals. This position is partially grant-funded and will
help to fulfill requirements of a contract with the North Dakota
Department of Health and Human Services, Aging Services Section.
Responsibilities include the facilitation of care consultation and
education, completing accurate and timely documentation, processing
referrals and collaborating with other organizations across North
Dakota. This Community Program Manager will serve designated
diverse communities across western North Dakota and other areas of
the state as needed.
This position is grant funded.
